New Patient Exam + Office Visit:
This is for first-time patients or if more than 24 months has passed since your last visit.
Patients should also select this treatment option if it your first visit after a car accident.
New Injury Exam + Office Visit:
This for existing patients with new complaints/injuries (excluding car accidents) since your last visit.
Follow-Up Visit:
This is for patients following their initial exam as well as for regularly scheduled visits.
Sports Physicals:
Please confirm that your physical can be completed by a Doctor of Chiropractic (or D.C.) as some sports physicals require M.D or D.O.
Please note that Sports Physicals are cash only and not billable to insurance companies.
